Local clinics designated by Blue Cross to serve Marlette Regional Hospital

MARLETTE — Marlette Regional Hospital announced last week that Blue Cross Blue Shield of Michigan has designated Family HealthCare of Brown City, Marlette Family HealthCare, and North Branch Family HealthCare as Patient- Centered Medical Home (PCMH) practices for 2017. The designation period runs from July 1, 2017 through June 30, 2018.

With the PCMH model of care, our family practices establish medical teams that work in partnership with our patients to keep them healthy and monitor their care on an ongoing basis. This is accomplished through technology like our electronic medical record system that helps our care team track and monitor patients’ conditions and offer up-to-date recommendations for preventive care and chronic disease management. With an enhanced focus on educating our patients and keeping them informed, patients learn to better manage chronic conditions like asthma and diabetes.
